Need a clarification to be ready to send passport when requested by CHC. My full name in passport is KPV...V is my first name and K family name. In the CIC file my first name is shown as K and Surname as V. In the checklist to be filled up with passport request should I mention V as first name and K as family name so that in future Canadian records my name shall appear as in the western style of VPK?

You will have to get a legal Name Change. There are some libraries that provide a notary service. One such is at Don Valley and Sheppard, If it is anywhere close to you. Please find the timings of their visit and present yourself and follow the procedure that they want you to follow. Might cost you a few bucks??
This is a legal (Quasi Legal) matter and hence the suggestion to follow.

Thanks for the reply. Please note that I am not a landed immigrant. I am in the final stages of processing, waiting for PPR. As I understand there is a check list to be filled up and sent with passports to CHC London. My question is relating to the name and surname to be mentioned in this checklist.

What I understood from your post is that in CIC records your first name and last name has been transposed .... don't you think your first step should be (or should have been) to have this mistake rectified....

Thanks for the reply. I have renewed my passports last month. Now in my new passport in the first row of Surname it is given as K...P.. and second row of Given name is V.... (which is right) In the medical requests my first name is given as K..P.. and surname is given as V... I have done my medicals and is waiting for PPR. Do you mean that I should sent a letter to CHC mentioning that my firstname should be V.. and surname be K....or is it enough that I mention this in the check list to be sent with Passports for Visa stamping. Kindly advise.
