Dear CDs,
My 10 months old daughter is in India on 1 year limited validity Canadian Passport. I would like to extend validity of my daughter's Canadian passport. I know that I will have to submit my daughter's Canadian passport and the fee of Indian Rupees 490 (to be sent as a demand draft in favor of the Canadian High Commission, payable at New Delhi).
My question is what kind of application forms do I have to fill in and submit to CHC Delhi along with documents and fees?

The following link will provide you with a PDF file and the ability to print and fill the required materials into the application.
You can also write and ask for the application forms for a child under 16. Also ask them if there is a simplified version. (We have the same for the adults here in Canada)
The fee I believe is $22
