Medicals not mandatory for visit visa ... irrespective of age. In most of the cases they do not ask. Particularly it works thatway If the applicant says that he or she is taking medical insurance.

If it is a first time visa, it is 100% sure that CIC delhi will ask for medicials irrespective of the health insurance that you will buy. HI helps to process the visa, if the medical finds some minor hitch like high BP, Colestrol.
I had the medicals skipped for my parents 2 times (they would have failed in the medicals) by routing them to enter Canada from USA. For this you need a USA visit visa.

In August my in laws got 10 years multiple .... No medicals. They are in early seventy's.

One more question, do i have to send old passport with new passport when apply for multiple visa this time or not?

It is not required. But it won't hurt if you say and enclose copy of old passport.

Looks like your parents do not live in India. Otherwise I am yet to hear of any parent located in India who has not been asked not to do the medicals. However there can be exceptions.
