The question is: My wife has all the documents (Study documents) from India with her First Name only. But when passport was made, she was asked to use "Last Name" of the family (This happened before marriage). Since in Canada, all documents/ID's use last name. This is causing the confusion that there was a name change. Some departments understand this scenario but some are more strict. What can be done to prove that this is the same person and there was no name change?

These things that you are telling us are common occurrences that take place in India. But things have changed immensely since then. The only way you can get them corrected is at the SOURCE, which is the Educational Institution and you have to contact them and get it rectified the way you want the Degree or the Document to read. That goes for the MARKS SHEET too. All of this is to make sure if and when they make an enquiry or want to cross-reference the same, it should match.
Some things that NO EDUCATIONAL INSTITUTE CAN DO is to change it to the MARRIED NAME. So, it will reflect it in their changes. Only a Marriage Certificate will prove her change of name and her new title. That can be accomplished here in Canada and there are procedures that you can follow and pay the required fees and do it.
Some of it if not required Immediately, then get it completed the next time you visit your native place/her native place. Then bring the same here and follow up to get her married name into the documents. It happens to Passports, Driver's licenses, Degrees,
Certificates also numerous other titles. So, please make a list of things that call for such changes and do the same carefully. In certain circumstances, a Notarization will help too. You will have to find out what will help.
