Here is a sample of a document. You will have to use this format. To do that you have to modify to suit your location. Since you are the only person who needs it, please eliminate the female section. Your would be wife may also have to obtain one at the Indian end. Consider that as a possibility and proceed in that direction. She can omit the male portion of the application.

If you were born in Canada, a certified copy of your birth certificate.
If you were not born in Canada, certified true copies of your Canadian citizenship card and Record of Landing form.
A statutory declaration of your present marital status and the name and citizenship of your future spouse. (Means that you say that you were never married)
A certified copy of a divorce certificate or divorce judgment if divorced or a death certificate of a previous spouse if widowed. (If it applies to you)
You will have to mail it to them with a reply paid envelope and wait to receive it. Also contact them to find out if there are any costs associated with it.
