Don't know about Home Phone but there is lots of advertising about Rogers Home Phone Challange. They are comparing their home phone @ $25 with Bell's @ $40.
Internet I've been using ACANAC and its a good service for the price (4-5 mbps) for $18 if u have home phone line else $26 all inclusive. They've leased it through Bell, so u'll actually get Bell's internet. But the speed depends on the area you live. Rogers have some promos going on for 3 month free and $100 credit for internet. So overall avg will comeout to be $40 with tax and Hi speed (10mbps) or you can choose lower speed and save there too.
TV, well FTA are not always there, if it goes off you never know when it'll come back. You might have to buy a new FTA for atleast $100. If u really need TV then buy the service else don't take anything. Its all needs and wants.
Long Distance, there are many services. Choose which best suits you. People above me have listed some good options.

a. Bell ....
2mbps DSL
Analog Home Phone + 2 calling features
1 year contract $ 52 / month .....
b. Acanan & Primus
5mbps acanac line paid up front for the whole year : 25 / month
Primus analog home phone : $25 / mo for 1st year ... $30 / mo later
Cancellation charge $60/-
Total 50 + 2 (modem ) = 52 / month ....
Appreciate your advice ... to me its a tie ....

I am using Reliance iCall for the last 3 weeks and I am happy with the quality. It is only 1c USD per minute to land-line/cellphone in India.

On Saturdays the quality is very good. On Sundays, rarely it is a bit weak. I use my headset (this will reduce echo-effect) and my handsfree microphone (in my web-cam). The only thing is you may need to speak a bit louder.
The voice on the other side comes crystal clear.
